Twice-Yearly Depemokimab Reduces Rescue Use in Recurrent CRSwNP, With Josesph Han, MD

 in Philadelphia from February 28 – March 2, Joseph Han, MD, from Old Dominion University, highlighted new data from a post hoc pooled analysis of ANCHOR-1 and ANCHOR-2 evaluating twice-yearly depemokimab in patients with 

chronic rhinosinusitis with nasal polyps (CRSwNP)

 marked by type 2 (T2) inflammation and disease recurrence following surgery.

Depemokimab, an ultra–long-acting anti–IL-5 biologic, is designed for administration every 26 weeks, offering sustained suppression of eosinophilic inflammation. The analysis focused on a clinically relevant subgroup: adults with ≥ prior nasal polyp surgery and ≥ 1 T2 inflammatory feature, including blood eosinophils ≥150 cells/μL, comorbid asthma, or NSAID- or aspirin-exacerbated respiratory disease. In real-world practice, this is the population most likely to be considered for biologic therapy.

A total of 154 patients were included in the pooled analysis (79 receiving depemokimab and 75 receiving a placebo). Across multiple endpoints, depemokimab demonstrated higher responder rates compared with placebo at 52 weeks.

For nasal obstruction, assessed using the verbal response scale, 46% of patients receiving depemokimab achieved ≥a 1-point reduction, compared with 21% with placebo (odds ratio [OR] 2.82; 95% CI 1.37–5.78; 

 =.006). Improvements in loss of smell (≥ 0.9-point decrease) were observed in 29% versus 15% (OR, 2.35; 

Clinically meaningful improvements in health-related quality of life, measured by SNOT-22 (≥ 8.9-point decrease), occurred in 65% of treated patients versus 43% on placebo (OR, 2.25;

 =0.019). Nasal polyp score response (≥ 1-point decrease) was achieved in 41% versus 25% (OR 2.04; 

Rescue intervention use, defined as the need for systemic corticosteroids or additional surgery, was significantly lower with depemokimab: 19% compared with 37% in the placebo arm (OR, 0.34; 95% CI 0.16–0.75; 

 =.008). According to Han, this reduction in rescue interventions reflects better control of underlying inflammation and fewer acute exacerbations requiring escalation of care.

During the interview, Han discussed the clinical implications of a twice-yearly treatment for CRSwNP. He cited market survey data evaluating patient preferences for medication delivery, including whether patients would favor a subcutaneous injection every 6 months over a daily oral pill.

“What this tells us is that… patients would prefer [depemokimab every 6 months) rather than taking a pill every day,” Han said.

Shahriari emphasized that the phrase “skin health equity” was chosen intentionally. For today’s dermatologists, she noted, the conversation must extend beyond isolated diagnoses or cosmetic concerns to encompass outcomes, access, education, and trust. What once was discussed quietly “in the hallways” is now visible in the literature, on social media, and across industry initiatives. The shift, she suggested, is from performative concern to measurable accountability: clinicians, institutions, and companies are increasingly being asked to demonstrate what they are doing differently.

Taylor reflected on why this period feels distinct from prior waves of discussion. In her view, one of the most tangible differences is therapeutic progress in diseases that have historically and disproportionately affected patients with skin of color. She pointed to hidradenitis suppurativa (HS) and atopic dermatitis as examples in which clinicians now have more effective and safer treatment options than in years past. These therapeutic advances, she explained, are complemented by educational gains, including expanded image collections and textbooks that more comprehensively depict dermatologic disease across a spectrum of skin tones. Such resources are critical for improving diagnostic accuracy and reducing delays in care.

Elbuluk agreed that the landscape has evolved from dialogue to accountability. She observed that journals, conferences, pharmaceutical companies, and even the cosmeceutical industry are now explicitly prioritizing health equity. Whereas prior limitations, such as a lack of diverse representation in research or educational materials, might have been passively accepted, there is now an expectation that organizations address these gaps. According to Elbuluk, that expectation signals a readiness within the field to pivot from aspiration to implementation.

Shahriari also framed equity as a patient safety issue. As the US population becomes increasingly diverse, she argued, the inability to effectively diagnose and manage dermatologic disease across heterogeneous skin types risks compromising care. For practicing clinicians, this reframing underscores that equity is not ancillary to core dermatologic practice; it is foundational to delivering safe, high-quality care.

The discussion also addressed ongoing gaps. Taylor highlighted clinical trials as an area of both progress and unfinished work. She cited efforts to increase the diversity of both investigators and study participants, noting initiatives supported by professional organizations and pharmaceutical companies. Trials intentionally designed to evaluate therapies in patients with skin of color represent an encouraging step, but she stressed that sustained commitment is necessary to ensure study populations reflect real-world demographics.

Beyond research, Elbuluk pointed to changes in conference programming and educational content. Speakers are becoming more intentional about including diverse imagery in lectures, and sessions addressing conditions that disproportionately affect patients of color are more visible. Importantly, she noted that skin of color dermatology is increasingly integrated throughout meetings and curricula, rather than siloed as a standalone topic, an evolution that reinforces its relevance to all clinicians.

Collectively, the panelists conveyed cautious optimism. Meaningful gains have been made in therapeutics, education, and representation, but structural and clinical gaps remain. For dermatologists in practice, the message was clear: progress matters only if accompanied by honest appraisal and continued action to ensure equitable, evidence-based care for all patients.

The discussion begins with the phase 3 ACHIEVE-3 trial evaluating orforglipron, a novel, nonpeptide, once-daily oral GLP-1 receptor agonist, studied head-to-head against oral semaglutide in 1,698 adults with type 2 diabetes inadequately controlled on metformin. Orforglipron demonstrated superior glycemic efficacy, achieving mean A1C reductions of 1.7% and 1.9% at the 12 mg and 36 mg doses, respectively, compared with 1.2% and 1.5% reductions seen with 7 mg and 14 mg oral semaglutide. All primary noninferiority endpoints were met, with statistical superiority observed across comparisons. Weight reduction also favored orforglipron.

However, the enhanced efficacy was accompanied by higher rates of gastrointestinal adverse events. Nausea and other GI symptoms were reported in approximately 58–59% of patients receiving orforglipron versus 37–45% with semaglutide. Importantly, treatment discontinuation due to GI events remained relatively low (6% with orforglipron vs 3% with semaglutide), suggesting tolerability may be manageable in clinical practice. The hosts emphasize the importance of understanding how adverse events are captured in trials and highlight the role of dose titration, dietary counseling, and supportive medications in mitigating side effects. A key limitation noted is the absence of cardiovascular outcomes data for orforglipron, particularly given established cardiovascular benefit with oral semaglutide, underscoring the need for outcomes-driven decision-making in high-risk populations.

Attention then shifts to top-line results from the phase 3 TRIUMPH-4 trial evaluating retatrutide, a first-in-class triple agonist targeting GLP-1, GIP, and glucagon receptors, in individuals with obesity and osteoarthritis. Over 68 weeks, retatrutide achieved mean weight reductions of 26.4% to 28.7% (up to 71.2 pounds), representing some of the most substantial weight loss observed in pharmacotherapy trials to date. Nearly half of participants achieved ≥25% weight loss, and up to 23.7% achieved ≥35% reduction. Clinically meaningful improvements in osteoarthritis symptoms were documented via WOMAC scoring, with one in eight participants reporting complete resolution of knee pain. Gastrointestinal adverse events were dose-dependent, and discontinuation rates ranged from 12% to 18%, reinforcing the importance of balancing efficacy with tolerability.

The hosts also review early phase 2 data from Novo Nordisk’s investigational triple agonist (UBT251), which demonstrated 19.7% weight loss over 24 weeks in a Chinese cohort, along with improvements in waist circumference, glycemia, blood pressure, and lipids.
