New phase 3 data, pooled from the LIBERTY-PN PRIME and PRIME2 trials, suggest dupilumab is efficacious among patients both above and below the age of 65 years who live with prurigo nodularis.

These data were highlighted in a recent interview with Shawn Kwatra, MD, during the 

2025 European Academy of Dermatology and Venereology (EADV) Congress

. Kwatra is a renowned leader in the dermatology space, currently serving as a professor and chairman of dermatology at the University of Maryland School of Medicine.

“Prurigo nodularis is one of those diseases that robs people of their dignity. The Itching is relentless,” Kwatra explained. “The scratching is constant, and for many patients, especially our older patients with other medical conditions, it becomes a daily battle. So the question has always been, can we deliver a therapy that is both safe and powerfully effective across patient groups? In particular age groups, it is very important for us to know, since many of our prurigo nodularis patients actually start in middle age. But many are older adults.”

Adult patients with moderate to severe prurigo nodularis had been assessed in the LIBERTY-PN PRIME/PRIME2 studies (NCT04183335/NCT04202679).

 These subjects specifically had disease that was inadequately controlled via topical drugs or for whom those medications were shown to be inadvisable.

Kwatra and colleagues randomized these individuals to 300-mg dupilumab on an every-2-week basis (n = 153) or into the matched placebo cohort (n = 158) for a total of 24 weeks. At Week 24, the investigatorsa evaluated the proportion of individuals younger or older than 65 years who attained a ≥4-point Worst Itch Numerical Rating Scale (WI-NRS) score reduction or Investigator’s Global Assessment for PN-Stage (IGA PN-S) score 0/1.

“In the pooled analysis…dupilumab showed robust efficacy in prurigo nodularis,” Kwatra said. “This was a game changer. I've been studying prurigo nodularis for my career, and we didn't know if we'd ever have an FDA-approved therapy for prurigo nodularis. But the biology happened fast. The trials happened quickly, and we got that approval. Dupilumab led the way here with that first FDA-approved therapy for prurigo nodularis. At 24 weeks, what we found here is that both in PRIME and PRIME2, over 60% of patients, both over and under age 65, had at least a four-point reduction in their itch. That's significant.”

Kwatra highlighted the data in this analysis, noting that those given the treatment compared to those on placebo saw significantly greater proportion of patients aged <65 years (63.5% versus 23.1%; 

 < .0001) and ≥65 years (63.2% versus 25.0%; 

 = .0142) attaining a reduction of WI-NRS scores by ≥4 points from baseline.

The investigators also found that a significantly greater proportion of dupilumab-treated individuals aged <65 years (44.4% versus 19.4%; 

 = .0023) and ≥65 years (52.6% versus 4.2%; 

 < .0001) were successful in attaining an IGA PN-S score 0/1 at the 24-week mark.

 Safety was also shown by Kwatra and coauthors to be similar between these age cohorts. It was also noted to have been generally consistent with the established safety profile of dupilumab.

Prevalence of metabolic syndrome (MetS) is higher among patients with 

 compared to both the general population and other inflammatory arthropathies, according to a recent meta-analysis.

The correlation between MetS and PsA have been well known for years, with abdominal obesity, hypertension, hyperglycemia, and hyperlipidemia tied closely to both conditions. Although the pathophysiology of this link is still unknown, studies have not only emphasized this connection but also highlighted an association between PsA and the frequency of several of MetS’s components.

“An association between PsA and MetS has been suggested, with several studies examining the prevalence in PsA compared to other groups with somewhat diverging results,” Sara Andreasson, department of public health and clinical medicine, Umeå University, Sweden, and colleagues wrote. “The aim of this meta-analysis is to compile current knowledge on the prevalence of MetS and its individual conditions in PsA compared to the general population, PsO, and other inflammatory arthropathies.”

An initial total of 1526 articles were selected for screening by 2 independent researchers, who then sorted them based on relevance. Articles without original data, studies conducted in children, and articles with a patient sample collected via a dermatology clinic were excluded, among others. The remaining articles were then read in full, with those investigating the prevalence of MetS in patients with PsA compared to a control population included.

Investigators ultimately narrowed the search down to 20 studies. Of these, 7 studies examined the prevalence of MetS in PsA compared to the general population, 8 studies examined its prevalence in PsA compared to 

), and 7 studies examined it compared to psoriasis (PsO).

When compared to the general population, all but 1 of the 7 studies reported statistically significant increases in the prevalence of MetS in PsA. 6 of the 8 studies comparing PsA to RA noted a statistically significant increase in MetS prevalence in PsA. 2 of the 3 studies comparing PsA to AS showed a statistically significant increase. 4 of the 7 studies comparing PsA to PsO showed no statistically significant difference. Heterogeneity was low in the general population group, moderate in the RA group, and high in the AS and PsO groups. Homogeneity was statistically significant in the population group, the AS group and the PsO group, but not significant in the RA group.

The analysis comparing PsA to the general population indicated a 2.5-fold increased risk of MetS in PsA, while the RA comparison saw a 1.9-fold increase. The PsO comparison showed no significant difference in MetS prevalence. The AS comparison saw no firm conclusion, as this cohort included only 3 studies.

Ultimately, investigators believe a correlation exists between PsA and increased MetS prevalence. Many of the included studies showed the correlation as associated with disease and inflammatory activity, suggesting a common underlying pathophysiology between the 2 conditions.

“It is therefore important to monitor and treat cardiovascular risk factors in these patients, while ensuring that patients achieve minimal disease activity,” Andreasson and colleagues wrote. “The impact of disease activity, inflammation, and antirheumatic drugs on the prevalence of MetS in PsA is incompletely understood, and more research is needed in this area on how to best manage the cardiovascular risk in these patients.”

Psoriatic Arthritis Strongly Associated with Metabolic Syndrome and Components

Dexpramipexole significantly improved lung function compared to placebo as an add-on oral therapy in people with eosinophilic 

, according to the latest findings from the phase 3 EXHALE-4 study announced by Areteia Therapeutics.

“More than half of people with asthma have the eosinophilic subtype, yet there remains a profound need for easily administered oral treatment options that can help address their symptoms, which are often severe,” Ian Pavord, MA, DM, Professor of Respiratory Medicine at the University of Oxford, United Kingdom, and member of Areteia’s Scientific Advisory Board, said in a statement.

 “Initial results from the Phase III EXHALE-4 study are extremely promising, particularly the statistically significant improvement in lung function and the reduction in eosinophils observed with dexpramipexole versus placebo, which show the potential for dexpramipexole to become the first oral treatment approved for eosinophilic asthma.”

EXHALE-4 is a randomized, double-blind, placebo-controlled Phase 3 study evaluating dexpramipexole, an investigational oral small molecule. It is formulated as dexpramipexole dihydrochloride and 75 mg and 150 mg dexpramipexole dihydrochloride are equivalent to 56 mg and 112 mg dexpramipexole, respectively (doses mentioned refer to dexpramipexole dihydrochloride).

The therapy is proposed to inhibit the maturation of eosinophils in the bone marrow, based on evidence from cell cultures and human biopsies, thereby lowering peripheral blood and tissue eosinophil levels.

The study is evaluating dexpramipexole in 600 participants aged ≥12 years with inadequately controlled moderate-to-severe asthma and blood Absolute Eosinophil Count ≥300 cells/μL. They continued their usual asthma medications and were randomized to add either dexpramipexole (150 mg or 75 mg) or placebo in a 5:5:1 ratio, respectively. Participants were randomly assigned to placebo BID, 150 mg BID, or 75 mg BID dexpramipexole groups in a 5:5:1 ratio respectively.

The study is primarily evaluating the absolute change from baseline in pre-bronchodilator forced expiratory volume (pre-BD FEV

) averaged over Weeks 20 and 24 between 150 mg twice daily (BID) dexpramipexole dose and placebo. Dexpramipexole 75mg BID was included to assess eosinophil lowering effects in blood and pharmacokinetic/pharmacodynamic relationship.

Investigators found that dexpramipexole significantly improved lung function and met the study’s primary endpoint, with a statistically significant improvement in pre-BD FEV

in the dexpramipexole 150 mg BID group compared with placebo. Significant improvements in lung function were observed as early as Week 4. Dexpramipexole 150 mg BID and 75mg BID also significantly reduced blood AEC averaged over Weeks 20 and 24 compared with placebo. The therapy was well tolerated, with a safety profile similar to previous studies. More detailed data will be presented at an upcoming medical meeting.

Dexpramipexole previously demonstrated in a significant, dose-dependent reduction in blood AEC at all doses tested (37.5 mg, 75 mg, or 150 mg BID) compared with placebo in a Phase II study in participants with moderate-to-severe eosinophilic asthma with dose-dependent improvements in lung function and a well-tolerated safety profile in other EXHALE studies.

Dexpramipexole Oral Add-On for Asthma Significantly Improves Lung Function Over Placebo

A study showed that falls were common among participants with systemic lupus erythematosus (SLE), regardless of age.

“Multiple intrinsic factors appear to be contributing to these falls, including loss of balance, weakness, joint instability, and pain…” wrote study investigator Chelsea R. Perfect, MD, MPH, from Duke University School of Medicine, and colleagues.

 “Extrinsic factors likely contributed to the high prevalence of falls, with nearly two-thirds of study participants reporting that they tripped or slipped during their fall.”

SLE is believed to accelerate the aging process due to stress, hormone alterations, and inflammation. The accelerated aging can put patients at risk of geriatric syndromes, including falls.

In the Approaches to Positive, Patient-centered Experiences of Aging with Lupus (APPEAL) study, investigators assessed the burden of and risk factors for falls among patients with SLE regardless of age.

 They also examined what intrinsic and extrinsic factors contributed to the falls.

The team recruited 447 participants aged ≥ 18 years with validated SLE (40.9% aged ≥ 50 years, 91.7% female, and 82.6% Black) from the ongoing population-based Georgians Organized Against Lupus (GOAL) cohort in Atlanta. This sample included participants who had an APPEAL visit between October 8, 2019, and May 12, 2022.

The median disease duration was 14.8 years, with median SLAQ and BILD scores of 11 and 2, respectively. The mean Short Physical Performance Battery score was 9.0, with balance and chair stand sub scores of 3.8 and 1.7, respectively. Most participants had mild limitations in basic and instrumental activities of daily living, with median scores of 6 and 9, respectively.

Investigators also collected self-reported information on falls, including the number of falls and fall-related injuries requiring medical attention over the past year. They also collected self-reported perceived contributing factors.

Nearly a third of patients (30.7%) reported falling in the past year, with 19.2% falling at least twice. Contributing factors for falls included loss of balance (78.1%), slipping or tripping (64.2%), and weakness (53.3%).

Neither age, sex, nor race was associated with falls. Although the study showed that participants aged ≥ 50 years were more likely to report falls than younger participants aged 18 – 34 years, this association was not statistically significant in the multivariable logistic regression analysis adjusted by age, sex, and race (adjusted odds ratio [aOR], 0.82; 95% confidence interval [CI], 0.22 – 3.07). The presence of cognitive impairment was also not associated with a difference in falls.

The study found that greater physical functioning (aOR, 0.50; 95% CI, 0.38–0.66) and performance (aOR, 0.78; 95% CI, 0.71–0.87) were associated with lower odds of falls. Risk factors associated with greater odds of falls included a greater SLE activity (aOR, 1.78; 95% CI, 1.44 – 2.21), greater SLE damage (aOR, 1.22; 95% CI, 1.00 – 1.49), and depressive symptoms (aOR, 1.43; 95% CI, 1.14 to 1.80). Additionally, obesity (aOR, 1.58; 95% CI, 1.04 to 2.41), greater Basic Activities of Daily Living scores (aOR, 1.29; 95% CI, 1.16 to 1.43), and greater Instrumental Activities of Daily Living scores (aOR, 1.14; 95% CI, 1.07 to 1.21) were linked to greater odds of a fall.

Another risk factor was taking medication that increases the risk of falls, such as antidepressants (aOR, 1.82; 95% CI, 1.13 – 2.93), pain medications (aOR, 2.60; 95% CI, 1.68 – 4.00), and opioids (aOR, 4.52; 95% CI, 2.39 – 8.56). Steroids, sedatives, and antihypertensive medications were not associated with increased odds if a fall. Taking ≥ 10 medications, compared with ≥ 5 medications, was associated with a greater risk of falls (aOR, 3.36; 95% CI, 1.83 to 6.16).

Furthermore, being currently employed versus unemployed was associated with a substantially lower odds of reporting falls (aOR, 0.61; 95% CI, 0.40 – 0.94). Additionally, high vs low physical activity was linked to lower odds of falls (OR, 0.33; 95% CI, 0.14 to 0.76).

Sensitivity analyses showed that falls attributed to balance, weakness, or slipping had distinct risk patterns: balance-related falls were more strongly linked to disease activity, depressive symptoms, stress, and medication use; weakness-related falls were influenced by education and race; and slipping-related falls were most strongly tied to employment, physical activity, and depression.

“Future studies could focus on understanding if there are differences between SLE individuals and the general population regarding severity of the falls, incidence of injury, and sequelae of falls,” investigators wrote.

Falls are prevalent among systemic lupus erythematosus (SLE) patients, irrespective of age, due to intrinsic factors like balance loss and extrinsic factors such as tripping. The APPEAL study assessed fall risk factors in SLE patients, revealing that greater physical functioning and performance reduce fall risk, while higher SLE activity, damage, depressive symptoms, and obesity increase it. Medication use, particularly antidepressants and opioids, also elevates fall risk. Employment and high physical activity correlate with reduced fall odds. Future research should explore fall severity and injury incidence in SLE versus the general population.

Nearly a third of patients with lupus in APPEAL reported falls, with disease activity, depressive symptoms, and medication use among key risk factors.

New research is shedding light on the benefits of GLP-1 RA use among people with 

, highlighting improvements in disease activity and cardiovascular risk profile.

Findings from the single-center, retrospective, observational study of electronic health record data suggest GLP-1 RAs may be useful tools for individuals with RA based on observed decreases in RA disease activity, pain, and cardiovascular risk factors.

GLP-1 RAs have taken medicine by storm in recent years. Since semaglutide was first approved for the treatment of 

 in 2017, the agent has earned several additional indications for obesity, cardiovascular risk reduction, 

 in type 2 diabetes, and most recently, the treatment of 

metabolic dysfunction-associated steatohepatitis (MASH)

“Although [GLP-1 RAs] are increasingly used in the general population, there is a paucity of data on their clinical effects in patients with RA,” Veena Ranganath, MD, MS, co-director of the UCLA Masters of Science in Clinical Research Program and director of the Rheumatology Fellowship Musculoskeletal Ultrasound Training Program, and colleagues wrote.

“Specifically, their impact on RA disease activity and cardiovascular risk factors in this patient population remains poorly characterized.”

To address this gap in research, investigators conducted a single-center, retrospective, observational study using data from 2018 to 2024 for a cohort of patients with RA with a body mass index of ≥27 who were prescribed either semaglutide or tirzepatide. A total of 173 patients took the prescribed GLP-1RA of interest, forming the treatment group. A control group comprised 42 patients who were prescribed but did not take a GLP-1RA during the same period.

Patients were assessed at 3-month intervals for up to 1 year after prescription for RA disease activity, cardiovascular risk markers, and patient-reported outcomes.

At baseline, investigators noted the treatment and control groups did not differ significantly in terms of BMI, RA disease activity, disease-modifying antirheumatic drug use, or prednisone use. However, they called attention to differences in baseline pain visual analog scale (VAS) scores, prevalence of hypertension and diabetes, HbA1c and serum triglyceride values, and the number of White patients between the 2 groups.

Results showed the treatment group experienced significantly greater reductions in RA disease activity (mean change score, −0.03 vs +0.2; 

 = .03), VAS pain score (−0.6 cm vs +1.3 cm; 

 <.001), weight (−4.4 kg vs −1.2 kg; 

 <.001), total cholesterol values (−10.3 mg/dL vs +0.3 mg/dL; 

 = .04), and HbA1c values (−0.3% vs −0.01%; 

Within the treatment group, investigators observed significant reductions in erythrocyte sedimentation rate (−3.0 mm/hr; 

 = .004), C-reactive protein (−0.6 mg/dL; 

 = .004), low-density lipoprotein cholesterol (−7.3 mg/dL; 

 = .002), and triglyceride values (−16.4 mg/dL; 

 = .004), whereas there were no significant reductions in these variables within the control group. Of note, in the between-group mixed effects model fitted to all data, the change in these variables did not differ significantly between groups.

Of the 173 patients in the treatment group, 50 (29%) discontinued the GLP-1RA during the study period, with the most common reasons for discontinuation being gastrointestinal adverse effects and insurance issues.

“Our study is among the first to assess the effects of GLP-1RAs on patients with RA, and our findings suggest that they may beneficially impact RA care in several important ways,” investigators concluded.

 “Further study in the form of prospective trials is needed to better characterize their benefits and risks in this patient population.”

New research indicates that GLP-1 receptor agonists (RAs) may benefit individuals with rheumatoid arthritis (RA) and obesity by reducing disease activity, pain, and cardiovascular risk factors. A retrospective study involving 173 patients with RA showed significant improvements in RA disease activity, pain scores, weight, cholesterol, and HbA1c levels among those treated with GLP-1 RAs compared to a control group. Despite these promising results, 29% of patients discontinued treatment due to adverse effects or insurance issues. Further prospective trials are needed to confirm these findings.

Compared with non-use, taking semaglutide or tirzepatide was associated with improved rheumatoid arthritis disease activity and cardiovascular risk.
