, according to an exploratory analysis presented at the 

2024 Association for Research in Vision and Ophthalmology (ARVO) Meeting

The analysis included nearly 1900 patients from the pivotal phase 3 YOSEMITE and RHINE trials and could reflect improvements in vascular stability identified with faricimab treatment determined in other biomarker analyses.

, investigator Roger A. Goldberg, MD, a vitreoretinal surgeon at Bay Area Retina Associates, described how patients with hard exudates exhibited no difference between the treatment arms after the loading phase. However, those treated with faricimab saw a notable reduction in hard exudates at the 52-week and 96-week marks.

“If you look at year 1 and year 2, we see about a 10% reduction in the portion of patients who have no lipid hard exudates in the faricimab-treated eyes versus the aflibercept 2 mg-treated eyes,” Goldberg told 

. “This suggests that faricimab, over time, does a better job at resolving hard exudates.”

Study participants were randomized 1:1:1 to receive intravitreal injections of faricimab 6.0 mg every 8 weeks (Q8W) or treat-and-extend (T&E) or aflibercept 2 mg Q8W. The presence of hard exudates was determined by a central reading center using color fundus photography within the ETDRS grid at screening and visits at 16 weeks, 52 weeks, and 96 weeks.

Evaluation for hard exudates was performed in 1870 patients from YOSEMITE and RHINE, with a similar proportion of patients with baseline hard exudates across all treatment arms (80.8 to 81.6%). Over time, the proportion of patients with hard exudates decreased during the study period.

Among those with hard exudates at baseline, the proportion at week 16 was similar between the faricimab and aflibercept treatment arms. At baseline to week 52, fewer patients treated with faricimab Q8W (79.0%) and T&E (86.2%) exhibited hard exudates versus patients treated with aflibercept (86.2%). This result remained at week 96 (52.8% and 55.9% vs. 64.5%).

Overall, these data corresponded to a difference of –7.2% (95% CI, –12.2 to –2.2; 

 = .0058) and –10.5% (95% CI, –15.6% to –5.4%; 

 <.0001) for faricimab Q8W and T&E at 52 weeks compared with aflibercept. Differences of –11.7% (95% CI, –18.6% to –4.8%; 

 = .0013) and –8.9% (95% CI, –15.7 to –2.1%; 

According to Goldberg, based on previous biomarker analyses, eyes with macular leakage may benefit from faricimab compared with other agents for DME. He noted these data indicate a similar biomarker-based decision to reach for faricimab in the presence of hard exudates.

“Similarly, I think clinically, when we see a lot of lipid hard exudates, that suggests this isn’t an eye with a lot of vascular instability and perhaps reaching for faricimab with its dual inhibition may be a better choice for these patients,” Goldberg told

Early-life usage of systemic antibiotics, especially during the first year of an infant’s life, impacts key gut microbiome components and is associated with a dose-dependent risk increase for 

Such findings are the conclusion of a recent study which was conducted with the goal of elucidating the timing and factors which impact systemic (oral or intravenous) antibiotic usage and eczema’s relationship. This research was led by Courtney Hoskinson, BS, of BC Children’s Hospital’s department of pediatrics at the University of British Columbia, Vancouver.

Hoskinson and colleagues noted that despite the existence of research on overuse of antibiotics and increased atopic dermatitis risk, there are few studies on the functional changes occurring within infants’ microbiomes connected with antibiotic usage.

“Within this study, we leveraged the large-scale, longitudinal CHILD birth cohort to investigate the mediating role of early-life exposure to systemic antibiotics in the previously established relationship between (lower respiratory tract infections) and subsequent AD development,” Hoskinson and colleagues wrote. “We further interrogated the metagenomic microbiome profiles of nearly 1,450 infants using stool collected at 1 year to mechanistically link antibiotic disruption and elevated AD risk.”

The investigators conducted the CHILD Study as a longitudinal, prospective birth cohort trial which lasted from 2008 - 2012. The research team enrolled 3621 pregnant women from 4 specified Canadian cities and included their infants (n=3455).

The infants looked at in the study had no congenital abnormalities. Participants involved in the CHILD study were monitored by the research team longitudinally, and detailed data was collected on environmental exposures as well as clinical outcomes through the use of questionnaires and in-person evaluations.

The team gave these questionnaires on environmental exposures and general health to the subjects at several different points from prenatal stages through to age 5, adding additional evaluations at ages 1, 3, and 5 years. The investigators considered several different covariates, including study center, birth weight Z-score adjusted for gestational age, the sex of the child, mode of delivery, season of child’s birth, breastfeeding status, presence of siblings, tobacco smoke exposure, and ethnicity.

The research team identified the child’s ethnic background through parental self-reports. The connection between subjects’ usage history of antibiotics, early-life changes in gut microbiome, and atopic dermatitis (AD) at age 5 was evaluated through use of differential analyses and statistical modeling.

The team looked at respiratory infections via the use of parent-completed questionnaires at different follow-up visits, with lower respiratory tract infections (LRTIs) being defined by specific symptoms. Overall, the team evaluated systemic antibiotics’ role in the relationship between early-life LRTIs and subjects’ development of clinical eczema at age 5, with a Generalized Linear Model (GLM) being used to calculate propensity scores.

The investigators reported that the use of systemic antibiotics within the initial year of life, as opposed to later years, was found to be associated with greater risk of atopic dermatitis development. This was indicated by an adjusted odds ratio (aOR) of 1.81 [95% CI = 1.28 - 2.57], 

Eczema risk was shown to rise depending on the number of antibiotic courses, indicating dose-dependent results. The research team also noted that shifts in participants’ microbiomes, which are related to systemic antibiotic implementation and eczema, explain the effect of antibiotic use on development of the skin condition (β

The shifts the investigators noted in the gut microbiome of the infants at 1 year old who later developed eczema, included greater levels of Tyzzerella nexilis and enhanced monosaccharide utilization. They also included decreased levels of Eubacterium spp., Bifidobacterium, and fermentative pathways.

“This study provides insight into the microbial underpinnings driving the link between early-life respiratory infections, antibiotic usage and the later development of AD,” they wrote. “This work reinforces the necessity of responsible antibiotic usage in pediatric populations and provides potential targets for the prediction and prevention of AD.”

Psychedelics do not seem to be leaving the research space any time soon. Not only are psychedelics being assessed for

 treatment but also for the treatment of substance use disorders (SUDs).

Brian Barnett, MD, from the Center for Behavioral Health, Neurological Institute at Cleveland Clinic, presented the history and challenges surrounding using psychedelics in treating SUDS, along with how psychedelics could potentially fit into the existing SUDs treatment paradigm, at the annual 

American Psychiatric Association (APA) conference

Back in the 1950s and 1960s, investigators did extensive research on psychedelics for the treatment of substance use disorders. Patients also had psychedelics in a clinical setting since at the time an FDA approval was not needed.

At the time, alcohol use disorder was a big problem, and investigators had an interest in using LSD to treat it. Many trials took place, but they resulted in mixed results. A study found participants who were more prepared and took the psychedelics in a soothing setting, rather than a hospital room or tied to a bed, had better outcomes.

Despite some studies suggesting psychedelics can improve SUDs, ethical concerns remain. In an interview with HCPLive, Barnett posed the question, “Can we give someone who already has a substance use disorder a psychoactive substance… that also has misuse potential?”

that out of all the classes of psychoactive drugs humans use, psychedelics have the “least addicting potential.”

He explained they are the least addictive for several reasons. The first one is that psychedelics are not enjoyable.

“Sometimes people associate psychedelics with euphoria, but for many people, that doesn’t happen,” he said. “They’re not euphoric; they actually find it to be quite a challenging experience.”

Another reason psychedelics are the least addictive is because they cannot be used for more than 3 days due to the property it has called Tachyphylaxis. If a psychedelic is used for 3 days, then it cannot be used for several days afterward.

“So that’s sort of a built-in protective feature,” Barnett said.

Additionally, research suggests psychedelics have serotonin 2c receptor agonists that activate serotonin receptors which ultimately reduces dopamine in the reward pathway. Since the dopamine in that area is what drives addiction, patients are less likely to get addicted to psychedelics.

Regardless, Barnett said clinicians should still warn their patients about the addiction risk. He also said clinicians should warn patients about the potential long-term personality or political changes, as well as other risks: suicidal ideation, psychosis, and for people with bipolar disorder, risk of mania.

“There are a lot of potential risks that we would have to talk about as we continue to gather more data to help us figure out who’s actually at risk for these things,” Barnett said. “We want to start out being as comprehensive as possible until we have more data to guide those discussions.”

Findings from a recent study are sounding the alarm on significant sex and ethnic disparities in 

Results of the multinational, real-world study were published in 

and showed female treatment-eligible patients were about 50% less likely to receive antiviral treatment and Asian patients from Western regions were also about 50% less likely to receive adequate evaluation or treatment compared to Asians from the East.

"It is vitally important to understand what the continual barriers are to treatment for patients with HBV,” Mindie Nguyen, MD, professor of medicine at Standford University, said in a press release.

In 2016, the World Health Organization set a goal to eliminate viral hepatitis as a public health problem by 2030 through a 90% reduction in incidence and a 65% reduction in mortality by 2030. An estimated 254 million people were living with chronic HBV in 2022, with 1.2 million new infections each year. Although it is preventable with a vaccine, linkage to care with oral antiviral therapy is necessary to suppress HBV replication and reduce hepatic inflammation, fibrosis, decompensation, and hepatocellular carcinoma (HCC) development.

To determine the proportion of patients with chronic HBV who received adequate treatment evaluation, were eligible for treatment according to American Association for the Study of Liver Diseases (AASLD) criteria, and initiated treatment with nucleos(t)ide analogs, investigators conducted a cross-sectional study within the Real-world Effectiveness from the global Alliance for HBV (REAL-B) registry, a retrospective multinational clinical consortium. Study data included treatment-naïve patients with chronic HBV from 25 REAL-B centers from 2000 - 2021.

For inclusion, patients were required to be ≥ 18 years of age, have a positive HBV DNA or hepatitis B surface antigen test for ≥6 months, and be treatment-naïve at the initial presentation. Patients with HCC, decompensated cirrhosis, organ transplant or other immunosuppression, or co-infection with hepatitis C or D, or HIV were excluded. Patients were followed until loss to follow-up, death, or the end of the study period, whichever came first.

Investigators categorized patients by sex, ethnicity, and geographic location. Of note, patients were considered to be from the East if their study center was located in Hong Kong, Japan, Korea, Singapore, or Taiwan, whereas patients from the West were from study centers in Argentina, Romania, Spain, and the US.

In total, 12,566 treatment-naïve adults with CHB met the study criteria and were included in the analysis. Overall, the cohort had a mean age of 47.1 (Standard deviation, 13.8) years and the majority of patients were male (58.3%) and of Asian ethnicity (96.1%). By geographic region, similar proportions of patients resided in the East (50.7%) and West (49.3%), although investigators noted a significantly greater proportion of non-Asian patients were located in the West (99.4%) compared to the East (0.6%) and the East contained almost entirely Asian patients (>99.9% vs 92.2%; 

Overall, 73.3% of the cohort received an adequate HBV evaluation. Investigators pointed out that compared to patients without an adequate evaluation, those who had an adequate evaluation were older (47.6 vs 45.7; 

 <.001), more likely to be of non-Asian ethnicity (4.1% vs 3.2%; 

 .02) and located in the East (53.5% vs 42.8%; 

 <.001), and more likely to have cirrhosis (11.2% vs 2.0%; 

After adjusting for age, sex, cirrhosis, and ethnicity plus region, female sex was associated with adequate evaluation (adjusted odds ratio [aOR], 1.13; 

 = .004). Compared to Asian patients from the East, Asian patients from Western regions were 40% less likely to receive an adequate evaluation (aOR, 0.60; 95% CI, 0.55-0.65; 

 <.001), while there was no significant difference with non-Asian patients from Western regions (aOR, 0.95; 95% CI, 0.76-1.19; 

By AASLD criteria, 32.6% of the patients who underwent adequate evaluation were treatment-eligible, 83.3% of whom initiated treatment with nucleos(t)ide analogs. Compared to untreated patients, treated patients were older (48.6 vs 44.7; 

 <.001), more likely to be male (66.6% vs 52.1%; 

 <.001), more likely to be from the East (69.2% vs 52.3%; 

 <.001), and more likely to have cirrhosis (26.5% vs 15.1%; 

Further analysis revealed female patients were about 50% less likely to be treated compared to their male counterparts (aOR, 0.54; 95% CI, 0.44-0.66; 

 <.001). Additionally, compared to Asian patients from Eastern regions, Asian patients from Western regions were also about 50% less likely to receive nucleos(t)ide analogs (aOR, 0.54; 95%, CI 0.44-0.66; 

 <.001), while there was no significant difference for non-Asian patients from Western regions (aOR, 0.83; 95% CI, 0.46-1.47; 

Investigators outlined several potential limitations to these findings, including potentially inflated rates of adequate evaluation and treatment initiation in specialized clinics compared to the community setting, lack of generalizability to regions not included in the study, overrepresentation of Asian patients, and the retrospective nature of the study.

“Our multinational real-world study of patients with chronic HBV revealed that rates of treatment evaluation and initiation remain below optimal levels, even among patients with cirrhosis and patients from referral practices,” Nguyen concluded.

“The symptoms of ADHD need to go back to childhood,” Alder said in the interview.

He explained the diagnostic ADHD screening requires a significant onset of attentional, hyperactivity, or impulsivity symptoms in childhood. According to the Centers for Disease Control & Prevention (CDC), several inattentive or hyperactive-impulsive symptoms must be present before age 12 years for an ADHD diagnosis.

 The symptoms also must be present in ≥ 2 settings such as home, school, or work.

“You don't have to have a full diagnosis, but the roots of the disorder have to be in childhood,” he said.

At APA, Alder presented a session called “ADHD Across the Lifespan: Getting Up to Speed” on the DSM criteria for diagnosing ADHD and why most adults remain undiagnosed and untreated. The presentation highlighted the importance of correctly diagnosing a patient. The presentation also showcased how the DSM only includes the core symptoms of inattention and hyperactivity-impulsivity in the diagnostic criteria, excluding the associated symptoms that may appear if someone has ADHD alongside a comorbid psychiatric disorder.

The associated symptoms tend to be more episodic. Symptom examples include executive dysfunction, executive function, deficits, emotional dysregulation, and sluggish cognitive tempo.

“Those symptoms are not…in the diagnostic manual, but they're highly comorbid and actually quite impairing,” Alder said. “They tend to occur almost as common as some of the core symptoms in ADHD, and they tend to not be as responsive to some of the pharmacologic interventions, so it's important for clinicians to be able to recognize them, and they will require potentially other treatments.”

Since they are less responsive than pharmacologic strategies, ADHD treatments are often taken alongside behavioral interventions such as cognitive behavioral therapy.

Following the pandemic, more patients have been diagnosed with ADHD, leading to a psychostimulant shortage. Although more ADHD treatments are in the investigational stage, Alder believes the current medications are effective and there just needs to be improvement in access, diagnoses, and getting the right medication to patients.

“Adult ADHD is a diagnosis where people can and should get better,” he said.
