A new tool can enhance the reliability of diagnoses of migraine among children and adolescents in both clinical and community settings. The authors describe this fully structured diagnostic interview, developed at the National Institute of Mental Health for a community-based family study of headache syndromes and comorbid disorders. It includes a validated open-ended clinical history, modules with key symptoms for each of the major headache subtypes, and associated impairment, duration, frequency, course, and treatment.

Two drugs, topiramate and trazodone, show some evidence for effectiveness against episodic migraines among children and adolescents, according to this systematic review of the literature. But so do placebos, which cut the number of monthly headaches nearly in half in these 21 placebo-controlled trials. The authors include a long list of other commonly used drugs for which no solid evidence supports their use in children and adolescents.
