Photo Quiz: Achilles Tendon Rupture? Tendonitis? Bursitis? Or Something Else?

A 39-year-old man presents with right heel and lower leg pain that began during a basketball game the previous night when a player inadvertently landed on the back of his lower leg. The patient can walk, but with difficulty. There is an obvious asymmetry when the distal posterior lower extremities are compared. The right lower extremity has diffuse edema, and a palpable groove proximal to the heel is evident. A Thompson test is performed bilaterally, with some flexion of the right foot but less flexion than the left foot.

This can either be a partially torn achilles or a fully torn achilles with an intact plantaris tendon which is allowing for the plantarflexion with the Thompson test.
