This 1-year-old boy is being followed for developmental delays. His facial appearance is somewhat unusual. He has increased deep tendon reflexes with clonus in the legs and mildly limited extension in the elbows and knees. He has intermittent constipation, irritability after feeds, and persistent posturing after eating with tilting of his head to the side.
