On occasion, the standard 14-day course of intravenous antibiotics can be extended in adults with cystic fibrosis who have acute pulmonary exacerbations. This prospective study found that 21 days of IV antibiotics can improve symptoms, but does not improve short-term or lung outcome measures. The authors discuss weighing risk factors and the outcome of extending the course of treatment.

Providing in-hospital IV antibiotics for several weeks affects the health costs and quality of life for cystic fibrosis patients and their families. This systematic review found evidence from a single randomized clinical trial that suggests, in the short term, home therapy does not harm patients, entails fewer investigations, reduces social disruptions, and can be cost-effective. The authors discuss how to make the individualized decision to attempt home treatment.
