Patient-reported outcomes were associated with disease activity levels and sociodemographic characteristics among a cohort of patients with 

, clinically suspect arthralgia (CSA), and unclassified arthritis (UA), according to a study published in 

Patients with RA self-reported a higher health burden compared with those with UA or CSA. Additionally, the health-related quality of life (HRQoL) in patients with pre-RA was significantly lower compared with the general population.

As early treatment of RA improves key clinical outcomes, including joint damage and disease activity, it is important to identify patients “at-risk” for developing classifiable RA.

“Subjective perceptions of health status can be captured via patient-reported outcome measures (PROMs), which can quantify symptoms, functional ability and general health status,” wrote a group of investigators led by Barbara Torlinska, PhD, MSc, associated with the Centre for Patient-Reported Outcomes Research, Institute of Applied Health Research at the University of Birmingham in the United Kingdom. “The impact of RA on PROMs is well understood, and PROMs are included in the core set of measures for RA clinical trials. However, the extent to which PROMs are impacted in pre-RA phases is less clear.”

To evaluate the HRQoL, fatigue, depression, and function among newly presenting patients with RA, CSA, and UA, a cross-sectional analysis of baseline PROMs was performed using patients from the Birmingham Early Arthritis Cohort. These measures were assessed using the Patient Health Questionnaire (PHQ)-9, the EuroQol 5 Dimension (EQ-5D), and the Functional Assessment of Chronic Illness Therapy – Fatigue (FACIT-F). PROMs were compared among these patient populations as well as among population averages from the Health Survey for England (HSE). Associations between clinical and sociodemographic variables and patient-reported outcomes were determined using multivariate linear regression.

Among a total of 838 patients included in the study, 484 had RA, 200 had CSA, and 154 had UA.

Patients with RA self-reported worse outcomes for all PROMs compared with CSA or UA. However, the mean EQ-5D utilities were slightly higher in CSA (.65; 95% confidence interval [CI]: .61 — .69) and UA (.61; 95% CI .56 — .66) compared with RA (.47; 95% CI .44 — .50). These results were lower than the general population and among older (≥ 65 years) participants.

For those with CSA and UA, HRQoL was comparable to other chronic conditions such as mild angina, severe chronic obstructive pulmonary disease (COPD), or heart failure. Patients with a higher body mass index (BMI) or older age (≥ 60 years) were more likely to have worse depression (PHQ-9: -2.47 [-3.85 — -1.09], 

 <.001) and fatigue (FACIT-F: 5.05 [2.37 — 7.73], 

 <.001). Female patients were more likely to report worse function (HAQ-DI: .13 [.03 — .21], 

 = .01) and fatigue (FACIT-F: -3.64 [-5.59 — -1.70], 

 <.001) compared with males. Further, people living in more deprived areas had decreased function (HAQ-DI: .23 [.10 — .36], 

 = .001), worse depression (PHQ-9: 1.89 [.59 — 3.18], 

 = .004), and fatigue (FACIT-F: -2.60 [-5.11 — 0.09], 

After investigators adjusted for confounding factors, disease activity and polypharmacy were linked to poorer performance across all PROMs. However, the diagnostic category was not associated with PROMs.

Investigators mentioned the recruitment of CSA patients based on clinical opinion as a limitation of the study. Additionally, the cross-sectional study design left them unable to assess a potential relationship between patient-reported burden at baseline to disease status at follow-up. 

“Our study shows that decreased functional status and compromised health outcomes are reported in individuals at risk of RA,” investigators concluded. “Studies of pharmacological intervention in the at-risk stages should therefore assess the impact of the intervention on these important patient-reported outcomes.”

In particular, the retrospective analysis observed improvement in the estimated glomerular filtration rate (eGFR) slope in 80% of patients with GLP-1 agonist treatment and all patients during SGLT2 inhibitor treatment.

“Based upon our results in a small cohort of SCD patients with T2D, the effects of these newer agents on kidney function should be further investigated,” wrote the investigative team, led by Santosh L. Saraf, MD, division of hematology and oncology, University of Illinois Chicago.

Acute and chronic organ damage drive early mortality in SCD—chronic kidney disease (CKD) occurs in nearly half of adults with SCD and a rapid decline in eGFR signals an increased risk of progression to end-stage kidney disease.

 Evidence on renin-angiotensin-aldosterone system (RAAS) blocking agents to treat SCD nephropathy is limited to short-term data with mixed results on kidney function preservation.

Large-scale clinical trials have proven the effect of GLP-1 agonists and SGLT2 inhibitors on improving adverse cardiovascular and renal outcomes. These benefits are independent of blood glucose lowering effects in patients with T2D and CKD, indicating these therapies' potential alternative reno-protective mechanisms.

The efficacy of both agents on kidney function and their safety on SCD-related complications, including vaso-occlusive pain episodes (VOEs), remains unknown in current literature. Saraf and colleagues retrospectively assessed adults with SCD treated at their institution who were prescribed GLP-1 agonists or SGLT2 inhibitors as outpatients.

For the analysis, the 2021 creatinine-based CKD Epistemology Collaboration (CKD-EPI) equation was used to calculate CKD and VOEs were defined as pain episodes requiring care in the emergency room, acute care center, or inpatient hospital setting. The eGFR slope was calculated using linear mixed-effects models, with the fixed effects defined as population-level baseline eGFR and time.

Between February 2016 and April 2023, 7 patients with SCD were treated with either a GLP-1 agonist or SGLT2 inhibitor at the investigators’ institution. Patients had a median age of 55 years, 5 were female, and all 7 had hemoglobin SS diseases. The median baseline eGFR before treatment was 76 mL/min/1.73m

 and the median urine albumin-to-creatinine ratio was 141 mg/g creatinine.

Among 5 patients treated with a GLP-1 agonist, the median duration of treatment was 21.8 months with liraglutide or dilaglutide. Serum glucose concentrations improved from a median of 213 mg/dL before treatment to 152 mg/dL during treatment. Rates of VOE increased from 1.0 per year to 2.1 per year during GLP-1 agonist treatment.

Upon analysis, the eGFR slope improved from a median of –2.37 mL/min/1.73 m

 per year before treatment to +0.49 mL/min/1.73 m

 per year during treatment. Four of 5 patients had an improvement in eGFR slope with a median change of +2.36 mL/min/1.73 m

Among 3 patients treated with an SGLT2 inhibitor, the median duration of treatment was 12.6 months with empagliflozin. Serum glucose concentrations were typically stable from a median of 124 mg/dL before treatment to 119 mg/dL during treatment. Rates of VOE increased from a median of 3.0 per year before SGLT2 inhibitor therapy to 6.2 per year after initiation of treatment.

Further, the eGFR slope improved from a median of –0.55 mL/min/1.73 m

 per year before treatment to +4.70 mL/min/1.73 m

 per year during treatment. All three patients on SGLT2 inhibitor therapy experienced an improvement in the eGFR slope, with a median change of +5.30 mL/min/1.73 m

“Future prospective studies will be needed to confirm the benefits on kidney function, as well as the potential risks on SCD-related complications, such as VOEs,” they wrote.

Obesity is substantially associated with clinician- and patient-assessed 

 disease severity, according to recent findings, though the investigators acknowledge the corresponding effect sizes indicate limited clinical relevance.

These findings represented the conclusion of a prospective, observational cohort study known as TREATgermany. This research was led by Stephan Traidl, MD, of Hannover Medical School’s department of dermatology and allergy in Hannover, Germany.

Traidl et al. noted that there was still widespread division among epidemiological 

 as far as arriving at a definitive conclusion on obesity’s link with increased severity of atopic dermatitis.

 The investigators added that some studies in Europe contradict the results of American studies on the topic.

“The purpose of this study was to investigate associations between obesity and subjective as well as physician-reported measures of disease severity in addition to comorbidities in AD patients,” Traidl and colleagues wrote. “For this purpose, real-life, reliable data from the TREATgermany registry, one of the largest AD registries worldwide, were analysed.”

The investigators designed the TREATgermany registry as a prospective observational cohort study which aimed at subjects with moderate-to-severe atopic dermatitis. In 2023, their cohort exceeded 1800 individuals recruited across 69 total recruitment centers. A prior outline explaining the registry's criteria for subject inclusion, methodologies, and goals was acknowledged.

In short, the research team summarized that adults in the age range of ≥18 years would have to have been diagnosed with eczema using the UK Working Party criteria for diagnosis, with identification occurring through objective Scoring Atopic Dermatitis (oSCORAD) evaluations >20. Subjects would also have to have undergone systemic therapy for atopic dermatitis within the previous 24 months.

Following their first interactions with TREATgermany participants, the team assessed subjects’ baseline characteristics and implemented these qualities to stratify based upon their reported Body Mass Index (BMI).

Specifically, the investigators separated subjects into 3 cohorts: those who were labeled underweight (BMI < 18.5 kg/m2), those labeled normal weight/overweight non-obese (18.5 ≤ BMI < 30 kg/m2), and those labeled obese (BMI ≥ 30 kg/m2). They also assessed and recorded participants’ educational background, age, sex, smoking habits, age at the time of disease onset, employment status, marital status, and disease severity.

The research team looked at Eczema Area and Severity Index (EASI), SCORing Atopic Dermatitis (oSCORAD), and Investigator's Global Assessment (IGA). The team also evaluated allergological data, including subjects’ sensitization profiles, total IgE, and comorbidities like allergic rhinitis and bronchial asthma.

Overall, there were 1416 participants involved in the research, 16.5% of which were shown to have a BMI ≥30 kg/m2 that suggested obesity. The investigators reported that obesity was shown to correlate with lower education levels as well as an increased prevalence of cigarette-smoking.

Despite these findings, apart from these specific factors, the research team found that obese and non-obese subjects with atopic dermatitis had similar baseline characteristics. The team added that a BMI increase was associated with increased oSCORAD (adjusted β: 1.24, 95% CI: 1.05–1.46, 

 = .013) as well as POEM scores (adjusted β: 1.09, 95% CI: 1.01–1.17, 

The investigators showed that there were modest distinctions as far as participants’ overall oSCORAD between obese and non-obese individuals with atopic dermatitis. Additionally, the team found that allergic comorbidities had been shown to be similar across all arms of the study, with asthma being the singular exception due to its prevalence among obese individuals (

“One limitation of the presented study is the low number of underweight AD study participants,” they wrote. “This prevented any adequate analysis from being conducted on this subgroup. However, the overall sample size was sufficiently large and well-characterized to investigate obese patients.”

Obesity Linked to Atopic Dermatitis Severity, Pathophysiological Data Gaps Remain

Reviva Pharmaceuticals assessed brilaroxazine, a serotonin-dopamine system modulator, in the phase 3 RECOVER trial led by Laxminarayan Bhag, PhD. Brilaroxazine targets D2/3/4 and 5-HT1A/2A receptors as a partial agonist and 5-HT2B/7 receptors as an antagonist. The drug controls inflammatory cytokines, which may serve as a disease driver.

Participants on brilaroxazine 50 mg achieved a 10.1-point reduction compared to placebo in the PANSS score (-23.9 vs -13.8

 < .001), as well as had improvements in positive, negative, and negative marder symptoms; PANSS social cognition, excitement or agitation, PSP, and CGI-S. Participants on brilaroxazine 15 mg had improvements on all endpoints compared to placebo.

Ereshefsky suggests brilaroxazine may not just be effective for treating symptoms of schizophrenia—he said the drug is “likely to be useful” in other psychiatric conditions, such as bipolar disorder and ADHD.

“Mechanistically, something is quite different here,” he said.

More than that, brilaroxazine may treat conditions in areas other than the psychiatric field. Since brilaroxazine controls areas of neuroinflammation and damage caused by inflammatory processes, the drug can potentially treat individuals with brain disease. Ereshefsky said because of brilaroxazine’s mechanism, investigators would like to assess brilaroxazine for pulmonary diseases, fibrosis, arterial hypertension, and psoriasis.

Ereshefsky explained the team needs to explore the link between brilaroxazine and weight gain. The phase 3 trial showed there were a weak association between brilaroxazine and weight gain, unlike the phase 2 trial, thus assessing this in a long-term study is important.

Ereshefsky said he is looking forward to working on the second phase 3 trial, and although he hopes the results will replicate RECOVER, clinical trials are always a challenge.

“You can't bet 100% on anything in psychiatry working, but this, I think, is a solid drug,” Ereshefsky said. “And it's going to be an important addition to the armamentarium, so I'm hoping to see this drug get to patients in the real world soon.”

There were no reported disclosures for Ereshefsky.

A new tool for predicting patients’ risk of kidney failure and all-cause death may offer a promising alternative to the current benchmark risk prediction model for 

, consistently outperforming the kidney failure risk equation in external testing.

The predictive algorithm and digital dashboard, known as KDpredict, was designed to simultaneously predict kidney failure and death over 1-5 year time horizons and support holistic decision-making for patients with moderate to severe CKD while addressing shortcomings of the kidney failure risk equation.

“We’ve been able to show that KDpredict is consistently more accurate in predicting the risks of kidney failure and death in adults with moderate-to-severe chronic kidney disease than the risk prediction model currently in use,” Pietro Ravani, MD, PhD, professor and clinician scientist in the department of medicine and institute of public health at the University of Calgary, said in a press release.

Acknowledging the shortcomings of the current kidney failure risk equation, investigators sought to build a tool to provide risk predictions for both kidney failure, accounting for the competing risk of death, and all-cause death in adults with newly documented moderate to severe CKD. Of note, the study proposes a strategy for prediction modeling to adapt to local settings rather than a one-size-fits-all model.

The multinational, longitudinal, population-based cohort study included individuals with newly recorded chronic kidney disease stage G3b-G4, defined as estimated glomerular filtration rate (eGFR) 15-44 mL/min/1.73 m2. Population-based health data were linked to form 3 cohorts in Alberta, Denmark, and Scotland, with the Alberta data used to train KDpredict and tested against data from Denmark and Scotland.

The super learner algorithm selected the best-performing regression models or machine learning algorithms based on their ability to predict kidney failure and mortality with minimized cross-validated prediction error based on the Brier score. Prespecified learners, including age, sex, eGFR, albuminuria, presence of diabetes, and cardiovascular disease, were ranked and combined in an ensemble or the learner with the lowest cross-validated prediction error was selected.

The index of prediction accuracy, a measure of calibration and discrimination calculated from the Brier score, was used to compare KDpredict with the kidney failure risk equation. Outcomes included all-cause death and kidney failure, defined as maintenance kidney replacement treatment or eGFR of 10 mL/min/1.73 m2 sustained for more than 90 days, whichever came first.

In total, 67,942 patients from Canada, 17,528 patients from Denmark, and 7740 patients from Scotland were included. Among the cohort, the median age was 77-80 years and the median eGFR was 39 mL/min/1.73 m2.

Median follow-up times were 5-6 years in all cohorts. Kidney failure rates ranged from 0.8-1.1 per 100 person-years and death rates ranged from 10-12 per 100 person-years. Investigators noted KDpredict was more accurate than the kidney failure risk equation in predicting kidney failure risk. Specifically, the 5-year index of prediction accuracy was 27.8% (95% CI, 25.2% to 30.6%) for KDpredict versus 18.1% (95% CI, 15.7% to 20.4%) for the kidney failure risk equation in Denmark and 30.5% (95% CI, 27.8% to 33.5%) for KDpredict versus 14.2% (95% CI, 12.0% to 16.5%) for the kidney failure risk equation in Scotland.

In hypothetical clinical scenarios, investigators pointed out predictions from the kidney failure risk equation and KDpredict differed substantially, potentially leading to diverging treatment decisions. In one example, an 80-year-old man with an eGFR of 30 mL/min/1.73 m2 and an albumin-to-creatinine ratio of 100 mg/g would receive a 5-year kidney failure risk prediction of 10% from the kidney failure risk equation but would receive a 5-year risk prediction of 2% for kidney failure and 57% for mortality from KDpredict.

Investigators also noted individual risk predictions from KDpredict were accurate for both outcomes with 4 or 6 variables. Additionally, models retrained using older data provided accurate predictions when tested in temporally distinct, more recent data.

Investigators highlighted several potential limitations to these findings, some of which include the use of data from 3 countries with predominantly White populations, the use of albuminuria measurements limited to albumin-to-creatinine ratio or protein-to-creatinine ratio in the external testing cohorts, the static nature of the prediction tool, and the need for testing in a randomized trial to validate its usefulness.

“KDpredict is unique in its ability to provide accurate predictions of risk for both clinical outcomes in adults with this severity of CKD at the point of first onset, when a timely discussion should occur,” investigators concluded.

 “By presenting risk predictions of both kidney failure and death, KDpredict supports patient-centered care and holistic decision making.”
