Dr. Basavaraj asks Dr. Khabbaza about the long-term consequences of bronchiectasis and the factors driving exacerbations and hospitalizations. Dr. Khabbaza frames the goal of treatment as preventing irreversible destruction of lung parenchyma and airways, changes that cannot be reversed once they occur. In patients with severe disease who are undertreated with airway clearance, he explains, inflammation can escalate unchecked, giving bacteria more opportunity to grow and eventually pushing patients past a tipping point into exacerbation.
Once exacerbations begin, Dr. Khabbaza notes that outcomes diverge depending on disease severity and organism resistance. Many patients with severe disease, or those colonized with multidrug-resistant organisms, are more likely to fail outpatient therapy and require hospitalization. He compares this pattern to other chronic inflammatory lung diseases, explaining that each exacerbation can set patients back, and some never fully return to their prior baseline lung function. Airway clearance remains the central therapy for slowing this trajectory, he says, but patients with advanced disease, inadequate clearance, and difficult-to-treat organisms are the ones most likely to progress to irreversible airway changes and repeated hospitalization requiring intravenous antibiotics.
Dr. Basavaraj then turns to quality of life, asking Dr. Metersky which domains are most affected. Dr. Metersky responds that virtually every domain can be impacted once patients develop respiratory insufficiency. Shortness of breath limits their ability to perform daily activities or participate socially, he explains, while chronic cough itself is exhausting and often embarrassing, keeping patients from public places such as church, movies, or stores out of fear of being judged. Chronic infection and inflammation also produce persistent fatigue independent of the coughing itself, and cough frequently disrupts sleep. In Dr. Metersky's view, nearly any aspect of daily life can be affected by significant bronchiectasis.
